Please note: The Ivors Academy role is to provide support with an standardised form to ensure songwriters gather the complete details to make the per diem claim. For details about payment arrangements and individual record company terms and conditions, please contact your record company representative, or the person who invited you to the session.
The form collects the information you need to request your per diem claim.
You will be asked for details about yourself as a songwriter. This includes your CAE/IPI number, date of birth, and full address. These details help confirm your identity and make sure your per diem is paid correctly. The form also asks for specific information about the writing session for which you are claiming.
You can include up to 5x session in one form. You will need to complete a separate form for additional sessions, or if you are completing the form for different labels’ sessions.
If you are completing this form on behalf of a songwriter (for example, as their manager or agent), you must have their approval and consent. All information provided must be accurate and relate to the songwriter.
